Support coordination primarily focuses on individuals with complex needs, including those with disabilities, mental health challenges, or those experiencing crises. At ACF, our support coordinators are here to assist you with:
Support coordination is crucial to the National Disability Insurance Scheme (NDIS). It assists NDIS participants in establishing, understanding, implementing, navigating, and accessing the services and resources they require to achieve their goals and enhance their quality of life.
The process involves collaborating with NDIS participants, service providers, and community organisations to ensure participants receive essential support in healthcare, housing, education, employment, and social services. ACF Support Coordinators work alongside individuals, families, and communities to help them access resources and provide ongoing assistance with problem-solving and goal-setting.
Understanding participants situations, identifying their challenges, and leveraging their strengths.
Creating a tailored plan based on participants’ NDIS funding, support needs, and desired frequency of services.
Our goal is to provide comprehensive, tailored support to help you achieve your goals. From assessing your needs to connecting you with the right services and monitoring your progress, we’re here to ensure you receive the guidance and care you deserve.
involves understanding our participant’s situation by identifying their challenges and strengths.
Work with our participants to create a tailored plan that details their support needs, the level of support they prefer, and the frequency with which they wish to receive assistance from their NDIS funding. Our thorough support plans empower our NDIS participants to achieve their goals by addressing their needs.
By understanding their needs through individual assessments and support plans. We then refer them to suitable service providers and link them to appropriate services. Throughout this process, we strongly emphasise our participants’ rights to choice and control.
At ACF, we regularly check in with our participants to track their progress against the support and NDIS plans. If services are not working effectively, we can adjust the plan.
Support coordinators do not serve as advocates; however, our staff can offer a degree of advocacy to ensure that participants receive the services they are entitled to and request any necessary adjustments or improvements in those services.
At the Australian Care Foundation (ACF), we provide support coordination services under the National Disability Insurance Scheme (NDIS) and offer a multi-level approach to the Coordination of Support (COS). The NDIS designs these levels to accommodate the diverse needs of participants based on the complexity of their circumstances and the degree of support they require. The NDIS framework generally comprises three levels of support coordination, and ACF aligns its services with these levels.
Below is an overview of the levels of support coordination that ACF offers:
We provide three levels of support coordination tailored to participants’ unique circumstances:

At ACF, we deliver multi-level support coordination to ensure personalised assistance for everyone based on their specific requirements. The Australian Care Foundation (ACF) plays a pivotal role in supporting individuals and families by providing personalised support coordination tailored to each participant’s unique circumstances. ACF has a comprehensive understanding of the challenges faced by individuals with disabilities and those in need of care and assistance, which allows them to offer a high level of participant-centred and empathetic service.
Here’s how ACF can assist participants in support coordination:
ACF support coordinators work closely with participants to assess their specific needs, preferences, and goals. They help participants identify key areas where they need support, whether for daily living, mental health, employment, or educational goals. The team at ACF is skilled at creating individualised plans that are realistic and achievable.
One of the most critical functions of support coordination is ensuring participants can access a wide range of services. ACF has a network of partners with service providers across various sectors, including health, housing, disability services, and mental health resources. This allows ACF coordinators to link participants to services best suited to their needs.
Beyond simply connecting participants to services, ACF empowers them to become more independent. It works with participants to build their capacity to manage their needs, increase their knowledge of available services, and navigate the system more confidently. This is particularly important for participants with complex needs who may need ongoing support to develop self-management skills.
ACF is committed to providing culturally sensitive and inclusive support. They understand the diversity within the Australian community and ensure that their services are adaptable to their participants’ cultural, linguistic, and social backgrounds. This approach helps to build trust and ensure that participants feel heard, understood, and respected.
Support coordination doesn’t stop once the initial services are set up. ACF conducts regular check-ins to ensure participants receive essential services and meet their goals. If any issues arise, ACF proactively tackles problems, advocates for necessary changes, and adjusts the support plan.
